 The following issues can cause a semi auto handgun to fail to fire on the first shot. (Pull of the trigger)  Failure to load a round in the chamber  Failure to pull the trigger back the distance required to fire the gun.  The chambered round did not fire  The mechanical safety was not placed in the fire position  The slide did not go full forward to lock the chamber  The handgun was not gripped correctly  The magazine was not locked in the magazine well
