The following issues can cause a semi auto handgun to fail to fire on the first shot. (Pull of the trigger) Failure to load a round in the chamber Failure to pull the trigger back the distance required to fire the gun. The chambered round did not fire The mechanical safety was not placed in the fire position The slide did not go full forward to lock the chamber The handgun was not gripped correctly The magazine was not locked in the magazine well
